Immunization Reminders

To keep children in schools healthy, Georgia requires children to get vaccines against certain diseases. Please see the below reminders for required student immunizations. Please contact your child’s doctor to schedule the correct vaccines this summer.

If your student has received or will receive an updated vaccine, then your doctor or the health department should provide an updated Immunization Form 3231. If you are given an updated form, then a copy needs to be given to your child’s school.

7th Grade Immunization Requirements

All children born on or after January 1, 2002 who are attending seventh grade must have received one dose of Tdap vaccine and one dose of meningococcal conjugate vaccine to fulfill immunization requirements.

8th to 12th Grade New Student Immunization Requirements

Children who are new entrants to a Georgia school in grades eight through twelve must have received one dose of Tdap vaccine and one dose of meningococcal conjugate vaccine to fulfill immunization requirements.

11th Grade Immunization Requirements

Children 16 years of age and older who are entering or transferring into the 11th grade will need proof of a meningococcal booster shot (MCV4), unless their first dose was received on or after their 16th birthday.

Religious Exemption

In the state of Georgia, if there is a religious objection to the vaccination of a child, a completed affidavit on Form 2208 is required. After completion of the affidavit, the parent or legal guardian must have the form filed with the school or childcare facility the child will be attending as legal documentation of exemption from the required immunizations mentioned above.

Medical Exemption

Please note that medical exemptions are required to be updated annually on the 3231 immunization form.

Parents of students without health insurance, or those whose health plan does not cover these vaccines, may call their local health department and ask about getting no or low-cost vaccines. For more information, visit http://dph.georgia.gov/vaccines-children or call (800)-848-3868.
